It looks like I filed for OPT while my I-20 was in Terminated status.
It seems this happened because the school didn’t update it. Does this mean I’ve been out of status (overstaying) from that point on, and could this cause a problem when re-entering?

If you filed for OPT while your I-20 was in Terminated status, you’d be considered out of status from the point your I-20 was terminated. So if you leave the country before 6 months have passed and obtain a new I-20 — for example, to pursue graduate school — you should be able to re-enter.
